Who it suits / who should be cautious
Facial fat grafting takes a small amount of fat from your own body (mainly thighs or abdomen), purifies it, and places it into hollow areas of the face. Many people choose it because it uses your own tissue rather than a foreign material, and fat that takes hold can last a long time.
- People whose broad areas such as the forehead, temples or front cheeks are hollow, making the face look tired or older
- People who want one long-lasting approach rather than repeating filler again and again
- People who prefer to restore volume with their own tissue instead of a foreign material
- People who can stay in Korea for about 5–7 days and allow for 1–2 weeks of swelling
How Fat Grafting Works — Harvest, Purify, Inject in 3 Steps
Fat grafting happens in three main steps. How carefully each step is done affects graft survival (fat living and settling in) and how smooth the result looks.
The 3 steps
| Step | What happens | Key point |
|---|---|---|
| Harvest | Fat is gently drawn from the inner or outer thigh, abdomen and similar areas with a thin tube (cannula) | Done through a tiny opening (a few mm), so scarring is small. The goal is not removing a lot of fat as in liposuction |
| Purify | Blood, oil and fluid are removed by centrifuge, filtering or similar methods, leaving only living fat | Methods differ by clinic. It is hard to say any one method is always better |
| Inject | Placed in tiny amounts across several layers with a thin cannula | Avoiding clumps lets the fat get a blood supply, which helps survival and reduces lumpiness |
Uses and cautions by area
Fat is good at softly filling large areas. It is not suited to building a sharp shape, like a defined nose tip.
| Area | Typical goal | Cautions |
|---|---|---|
| Forehead | Round out a flat or sunken forehead | The area is wide, so a large amount may be needed. Too much can make it look bulging |
| Temples (sides of the forehead) | Fill hollow temples to soften the face outline | Many blood vessels here, so controlling the depth is important |
| Front cheeks / mid-face | Fill hollow front cheeks for a fresher, younger look | Too much can make the cheeks look bunched when you smile |
| Around the nasolabial folds | Support the hollow beside the nose to soften the shadow of the folds | If sagging cheeks are the cause, fat alone has limits |
| Under the eyes | Soften sunken under-eyes and dark shadows | Thin skin shows lumpiness easily. Done carefully with small amounts of fine fat |
| Chin tip | Improve a short or recessed chin | Less able to build shape than filler or an implant |
What Is Survival Rate? — Not All the Fat Stays
Transferred fat must connect to new blood vessels in its new spot to survive. Some is absorbed and lost in the process, and the share that survives is called the survival rate. It varies widely by study, area and body type, and it is generally planned on the basis that only part of the amount placed will remain.
- Most of the fat that will remain is usually settled by about 3 months after the procedure.
- Areas with little movement, such as the forehead and temples, tend to hold fat relatively well, while areas with lots of expression movement tend to hold less.
- Smoking, strong pressure or massage right after the procedure, and rapid weight loss all work against survival.
- Any shortfall is usually topped up with a second session 3–6 months later, and some people plan for two sessions from the start.
Fat Grafting vs Hyaluronic Acid Filler
The two share similar goals but have different characters. Depending on the area and your schedule, you may use just one, or both together.
| Item | Fat grafting | Hyaluronic acid filler |
|---|---|---|
| Material | Your own body fat | Hyaluronic acid gel product |
| Duration | Fat that survives lasts long-term (changes with weight and aging) | About 6 months to 1 year (varies by area and product) |
| Natural look | Good for softly filling large areas | Good for fine shaping and small touch-ups |
| Predictability | Absorption means the final result is seen at about 3 months | Visible right away and settles in about 2 weeks |
| Downtime | Swelling and bruising 1–2 weeks, plus bruising and tightness at the harvest site | Swelling 2–3 days, about 1 week if bruising appears |
| Reversibility | Cannot be dissolved. Reducing it takes extra procedures such as suction or removal | Can be corrected by dissolving with an enzyme (hyaluronidase) |
| Stay | 5–7 days recommended | 2–3 days |
Be careful with the terms “micro-fat” and “stem cells”
- “Micro-fat” refers to processing fat into smaller particles to place in thin areas, and the standard differs by clinic. What matters more than the name is which area it is used in and how it is placed.
- The phrase “stem cell fat grafting” often emphasizes cell components naturally found in fat. It is hard to treat as proof of a guaranteed survival rate or skin improvement, and treatments that culture cells separately require separate approval.
- Claims like “guaranteed ○○% survival” or “permanent in one session” are likely exaggerated, so judge them carefully.
What Happens on the Day
- 1In-person consultation and design (about 30 minutes to 1 hour): the doctor looks at your face, decides the areas, amounts and harvest site, and marks them with a pen. Your manager interprets.
- 2Tests and preparationif you are having sleep anesthesia, simple tests and fasting instructions are checked.
- 3Harvest (about 20–30 minutes)the needed amount of fat is drawn through a small opening in the thigh, abdomen or similar area.
- 4Purify and inject (about 40 minutes to 1 hour): the filtered fat is placed in small amounts across several layers of the face.
- 5Recovery and heading back (about 1 hour)after waking from anesthesia you get cold compresses and aftercare instructions, then go to your accommodation. If you had sleep anesthesia, do not travel alone.
Swelling and Bruising — Day by Day
Fat grafting causes more swelling than filler, and it lasts longer. The early look is not the final result, so it is important not to rush.
- Day 0–3Swelling is at its peak. The face may look puffy and bruised, and the harvest site also has bruising and tightness.
- Day 4–7The main swelling starts to go down. Light outings are possible with a mask and hat, and bruises fade to a pale yellow.
- Weeks 1–2Most of the major swelling and bruising settles, making daily life and makeup easier. It may still look slightly overdone.
- Months 1–3Lingering swelling clears, some fat is absorbed and the shape settles.
- After 3 monthsMost of the remaining fat has settled. From this point you can decide whether a touch-up is needed.
Sample Stay Plan (6 nights, 7 days)
We generally recommend a stay of 5–7 days. The plan is to spend the peak swelling period in Korea and fly home after a progress check.
- Day 1Arrive → in-person consultation and design. Rest at accommodation near the clinic
- Day 2Fat grafting procedure → cold compresses and rest at accommodation
- Days 3–4Peak swelling. Rest mostly at your accommodation and share swelling photos with your manager
- Days 5–6Light outings with a mask are possible. Progress check and harvest site dressing at the clinic
- Day 7Fly home. We explain aftercare and how to contact us after you return
When Flying
- Flying after a progress check is fine for most people, but cabin pressure changes may make the swelling feel slightly worse for a while.
- Drink water often on the plane and avoid alcohol. Be careful not to press on your face with a sleep mask or neck pillow.
- If you wear a compression garment on the harvest site, move your legs often during a long flight.
Before and After the Procedure — Including the Harvest Site
Before
- From 1–2 weeks before, cut back on medicines and supplements that make you bruise easily, such as aspirin, anti-inflammatory painkillers, omega-3 and vitamin E (do not stop prescription medicine on your own; tell us at your consultation).
- Smoking works against fat survival, so we recommend stopping for at least 2 weeks before and after.
- Avoid crash dieting, and bring a record of any previous filler, fat grafting or thread procedures (area, timing, product).
Face care
- Do not press or massage the treated area. Avoid facial massage and meridian treatments for at least 1 month.
- For about 2 weeks, avoid saunas, jjimjilbang, hot baths and heat-based treatments.
- Avoid alcohol and intense exercise for at least 1–2 weeks.
- For a few days, sleep on your back with your head slightly raised, and do not sleep face down.
- In strong sun outdoors, protect bruised areas with a hat and sunscreen.
Harvest site care
- The harvest site can have bruising and tightness for 1–2 weeks, and you may wear a compression garment as the clinic instructs.
- Do not soak the small openings in water for long until the date you are told, and follow disinfection instructions.
- If stitches are used, they are usually removed around day 5–7, or dissolving stitches are used. Please check before you leave Korea.
Normal Reactions and Symptoms to Report Right Away
Common passing reactions
- Swelling, bruising and tightness in the face and harvest site (most improve within 1–2 weeks)
- Volume that looks too full early on, and a slightly firm feel when touched
- Temporary asymmetry from uneven swelling on the two sides
Side Effects to Know About
| Side effect | Description |
|---|---|
| Lumpiness | Fat clumps or remains unevenly, leaving an uneven surface. Shows easily under the eyes where the skin is thin |
| Overcorrection | More remains than expected and the area looks puffy. It cannot be dissolved, so extra procedures such as suction may be needed |
| Absorption / asymmetry | More is absorbed than expected or a difference between sides remains, so a touch-up may be needed |
| Fat necrosis / calcification | Fat that does not survive can remain as lumps, hard masses or calcium deposits. The risk rises if too much is placed |
| Infection | Rare, but heat, pus or spreading redness needs treatment |
| Vascular complications | Very rare, but the most serious complication, in which fat blocks a blood vessel. Needs immediate action |
Contact us immediately if
- The treated area or surrounding skin turns pale white or becomes mottled purple
- Your vision blurs, or you have eye pain, headache or dizziness
- Pain or swelling gets worse day by day, or you notice heat or pus
Questions to Ask at Your Consultation
- Which areas of my face suit fat grafting, and which are better with filler?
- Roughly how much will be placed in each area, and where will the fat be harvested?
- How much absorption do you expect, and how likely is a second session?
- When and under what conditions is the touch-up done?
- What kind of anesthesia is used, and does the doctor I consult also do the harvest and injection personally?
- How do you correct overcorrection or lumpiness if it happens?
- How do you handle emergencies such as vascular complications?
- Given my schedule, on which day should I have my progress check before flying home?
- If a problem comes up after I return home, can I consult with photos?
How to Choose a Clinic
- Whether a board-certified plastic surgeon personally consults and also does the harvest and injection
- Whether they honestly explain absorption and the chance of a second session (rather than stressing only “permanent” or “guaranteed ○○% survival”)
- Whether they explain area by area whether fat suits you or filler or another method is better
- Whether they avoid recommending an excessive amount from the start and look at the balance of the whole face
- Whether they explain anesthesia management and emergency response for sleep anesthesia
- Whether they offer foreign-language support and review your progress by photo after you return home
