What is gum contouring — a dental treatment that tidies the gum line you show when you smile
Gum contouring is a dental treatment that reshapes the gum line so your teeth show at their natural length. It is used for a ‘gummy smile’ (showing a lot of gum when you smile), teeth that look short and square, or gum heights that differ between left and right. It is easy to think of it as simply cutting away gum, but depending on the cause, the right approach may be reshaping the gum only, reshaping the bone beneath the gum as well (crown lengthening), or reducing the movement of the lip rather than treating the gum at all.
Commonly used for these concerns
- You show a wide band of upper gum when you smile and end up covering your mouth
- Your front teeth look short and stubby, or your teeth are uneven in length
- Your left and right gum heights differ, so your smile looks asymmetrical
- You want to balance tooth proportions first, before veneers or crowns
Gummy smile causes — where treatment choice starts
Even for the same gummy smile, a different cause means a completely different treatment. Several causes often overlap, so the diagnosis is the most important step.
| Cause | Characteristics | Methods usually considered |
|---|---|---|
| Excess gum (insufficient tooth exposure) | The teeth have fully erupted but gum still covers them, so the teeth look short | Laser gum contouring; crown lengthening if needed |
| Bone position | The bone beneath the gum sits high toward the tooth, so if only the gum is cut it grows back up | Crown lengthening including bone reshaping |
| Overactive or short upper lip | Tooth length is normal, but the lip rises a lot when you smile | Gummy smile Botox (lip Botox) |
| Tooth position / upper jaw overgrowth | The front teeth sit too low, or the upper jaw itself has grown long | Orthodontics; in severe cases, orthognathic (double-jaw) surgery is considered |
Laser gum contouring — when only gum is covering the teeth
If there is enough distance between the gum and the bone near the tooth root and only thick gum is covering the teeth, the gum edge is reshaped with a laser or electrosurgical tool to tidy the line. It is done under local anesthesia, with relatively little bleeding and few stitches, and usually takes around 1 hour depending on the area.
- Advantages: relatively simple, quick recovery, easy to fit into a short schedule
- Good to know: if the distance to the bone is insufficient, the gum can grow back up
- Area: usually decided based on the 6–10 upper front teeth that show when you smile
Crown lengthening — when bone reshaping is needed
For teeth and gum to stay healthily attached, a certain distance (the biologic width, roughly 2–3 mm) must be kept between the gum edge and the bone. If the bone sits too high, even cutting the gum makes the body try to restore this distance, so the gum comes back down. In that case, crown lengthening is done: the gum is opened and the bone height is reshaped as well.
- The surgical area is larger than laser alone and stitches may be needed
- Stitches are usually removed or the area checked after about 1 week
- Swelling and discomfort can last a few days, so leave some room in your schedule
Gummy smile Botox and orthodontics/jaw surgery — when the gum is not the cause
When tooth and gum length are normal but the upper lip rises too much when you smile, a small amount of botulinum toxin can be injected into the muscles that lift the lip to reduce its movement. The procedure takes just a few minutes, the effect appears over a few days to 2 weeks, and it usually lasts about 3–6 months, so repeat treatments are needed. Too high a dose can make your smile look unnatural, so it is adjusted starting from a small amount.
How to check it is genuine
- Check that the product is opened in front of you
- Have the product name and dose (units) explained before the procedure
- Ask to see the empty vial or label after use
If the front teeth sit too low, orthodontics can move the teeth upward; if the upper jaw itself has grown long as a skeletal problem, jaw surgery is considered. Both take a long time and are hard to fit into a short trip, so many people only get a diagnosis in Korea and make a long-term plan.
Tidying the gum line before veneers
When you get veneers or crowns, an uneven gum line makes even a well-made restoration look uneven in length. That is why the gum line is often tidied first, tooth proportions are balanced, and only then is the restoration made.
- Setting the gum height first makes it easier to design natural veneer length and width proportions
- With laser-based work, you can move on to the next step relatively quickly once the gums settle
- If crown lengthening with bone reshaping is involved, it is safer to wait until the gum line settles (usually several months) before the final restoration
Comparing the methods — pain, recovery, stay & longevity
| Item | Laser gum contouring | Crown lengthening | Gummy smile Botox |
|---|---|---|---|
| Suited cause | Gum covering the teeth | High bone position as well | Overactive upper lip |
| Pain | Local anesthesia, mild discomfort for a few days afterwards | Local anesthesia; swelling and soreness possible for a few days | Just a pinch from the injection |
| Recovery | Gums settle in 1–2 weeks | Stitches out at about 1 week; gums settle over several weeks | Back to daily life right away |
| Recommended stay | About 3–5 days | About 7–10 days | About 1–3 days (2 weeks to see the effect) |
| Longevity | Long-lasting if it matches the cause; some regrowth possible | Long-lasting | Usually 3–6 months, repeat needed |
When causes overlap, treatments are sometimes combined — for example laser plus Botox, or gum tidying followed by veneers. The exact combination and schedule are decided after the examination.
Treatment steps & schedule — digital scan & simulation
- 1Day 1 — Examination, photos and digital scan, with an X-ray or CT to check bone position if needed. With your manager interpreting, you discuss how much gum you want to show and your desired tooth length.
- 2Day 1 — Using the scan data, the gum line and tooth proportions are simulated so you can preview them. If veneers are planned too, you review the final shape together using a model or mock-up.
- 3Days 1–2 — If there is gum inflammation, it is treated first with scaling or similar, then laser gum contouring or crown lengthening is performed.
- 4The day after the procedure to 1 week — Wound check, and stitch removal if you had crown lengthening. Light sightseeing that is not strenuous is possible during this time.
- 5Before you fly home — You receive your records such as photos and scans, and decide when to continue with veneers.
Recovery and daily life while traveling
- Day of the procedureOnce the anesthesia wears off you may feel throbbing, which is managed with prescribed medication. Avoid hot, spicy and hard foods.
- Days 1–3The gums may look white or red and can hurt when the toothbrush touches them. Care for the area gently and use the prescribed mouthwash.
- Weeks 1–2The gum surface settles and most everyday meals become comfortable. After crown lengthening, the stitches are removed during this period.
- Several monthsThe gum line gradually settles and the final shape is established. Restorations are planned from this point.
- Avoid alcohol and smoking for at least a few days — 1–2 weeks after crown lengthening (bleeding, delayed healing)
- Rest from saunas and intense exercise for a few days because of bleeding risk
- Avoid straws and forceful rinsing for the first few days, as they irritate the wound
- Flying is possible in most cases, but after crown lengthening we recommend flying home once the stitches are out
Aftercare at home and what to do if problems arise
- Gently brush along the gum line with a soft toothbrush, and restart flossing once the wound has healed
- Manage gum inflammation with a checkup and scaling every 6 months to 1 year
- For Botox, discuss your next visit in advance, timed to when the effect fades
- If you continue with veneers, take photos of your gums regularly to record any changes
If bleeding continues or swelling becomes severe, visit a dentist at home right away. Send photos and symptoms to your manager and we will get an opinion from the Korean dental clinic and decide how to respond together. Having your before-and-after photos, scans and treatment records makes care at your home dentist much easier.
10 questions to ask at your consultation
When you travel from abroad, consultation opportunities are limited. Jot down the questions below and your manager will organize and interpret them in advance.
- Is my gummy smile caused by the gum, the bone, the lip or tooth position?
- Is laser alone enough, or do I need bone reshaping?
- How many teeth will be reshaped, and by how much?
- Can I see the resulting line in a simulation before the procedure?
- Is there a chance the gum could come back down?
- Would adding Botox help? Which product and how many units?
- How many days do I need to stay in Korea, and how many visits are there?
- When should I get veneers after gum contouring?
- Will the dentist I consulted perform the procedure personally?
- What should I do if a problem arises after I go home, and what records can I get?
How to choose a dental clinic
- Does it explain the cause using smile photos, X-rays and so on?
- If the cause is not the gum, does it honestly recommend another method instead of gum contouring?
- Does it show the resulting line beforehand with a digital scan or simulation?
- Does it plan the next step, such as veneers, together with you?
- Does it offer foreign-language service and photo consultations after you return home?
