In a nutshell

UltheraThermage
In a wordLifts sagging by pulling deep tissue upwardWarms the skin layer to tighten it and improve firmness and texture
EnergyFocused ultrasound (HIFU)Radiofrequency (RF)
Working depthDeep, down to the fascia layer (SMAS), applied point by pointThe dermis, applied broadly over an area
Best forSagging jawline, double chin, sagging near the nasolabial folds, drooping cheeksFine lines, loss of firmness, enlarged pores, lines around the eyes
SensationA sharp zing at the target depthBrief heat with cooling
Eye areaLimitedPossible up to the eyelids with a dedicated tip
Put simply: if your concern is that the contour of your face has slipped downward, Ulthera is the better fit; if it's that your skin itself has become thin and crepey, Thermage is. If you have both concerns, many people have the two together.

Finding your match — a checklist

Closer to Ulthera

Closer to Thermage

If you have little facial fat and hollow cheeks, an aggressive deep lift can actually make you look gaunt. In that case the intensity and areas treated are adjusted, or other methods are added, so be sure to mention it at your consultation.

How it works — why results build over several months

Ulthera gathers ultrasound energy into points at a specific depth under the skin, creating heat. The tissue there contracts, and as it heals your body produces new collagen. Thermage uses radiofrequency to warm the skin layer broadly, tightening collagen fibers and prompting renewal.

Both treatments need time for your body to build new collagen. You may feel a little tightening right away, but the real change develops over 2–3 months. If you have an important event (such as a wedding), have the treatment 2–3 months beforehand.

Shot count and genuine tips — what determines your result

Having both together — “Ul-thermage”

If you're concerned about both sagging and skin texture, many people have both treatments on the same day (often called “Ul-thermage”). Usually the deep-layer Ulthera comes first, followed by Thermage for the skin layer. Because it takes longer, extra time is allowed for numbing cream and sedation care.

CombinationBest for
Ulthera aloneSagging jawline or double chin is the main concern; skin texture is fine
Thermage aloneFine lines, eye area or skin texture is the main concern; little cheek volume
Ulthera + ThermageBoth sagging and loss of firmness; you want one big annual treatment
Lifting + skin boosterYou want to improve inner dryness and skin glow along with firmness

Frequently asked questions

If you had to choose just one, Ulthera or Thermage?
If your main concern is a sagging jawline or double chin, Ulthera is often the better fit; if it's fine lines, the eye area or skin texture, Thermage often is. Send us your photos and we'll get a doctor's opinion for you.
When will I see results?
You may feel a slight tightening right after, and the real change develops over 2–3 months as collagen builds up.
How long do results last?
Usually around 6 months to 1 year, but it varies a lot with age, skin condition and shot count.
Does it hurt a lot?
With Ulthera you feel a sharp zing in areas close to the bone; with Thermage, brief heat. Numbing cream is applied, and if it hurts the intensity can be adjusted right away.
Can I fly the day after treatment?
Yes. Both treatments involve almost no downtime, so you can fly the next day.
Is it okay to have it in my 20s?
If sagging hasn't started yet, skin care or a lighter treatment is often a better fit than strong lifting. At your consultation, we'll first assess whether you need it now.
How are they different from Shurink or InMode?
Shurink is in the same ultrasound family as Ulthera, and InMode is a radiofrequency device. The principles are similar, but the devices, tips and shot setups differ, so they are not the same treatment. We'll compare them based on your goals and schedule.
Can I have fillers or Botox at the same time?
Often yes, but order and spacing matter. Usually lifting is done first and injectables afterward, or on a separate day. Your doctor decides after examining you.
How can I tell it's genuine?
On request, a new genuine tip is opened in front of you before treatment, and you can also check the number of shots used afterward.
