When liposuction alone isn't enough — who needs a body lift
Liposuction removes fat; it does not shrink stretched skin itself. If the skin still has elasticity, it tightens up to a degree after the fat is removed, but skin that has lost elasticity can look even looser once the fat is gone. Cutting away the extra skin and stitching it taut is what a body lift does.
- People whose arms, belly or thighs hang in folds after major weight loss (including bariatric surgery)
- People with sagging lower-abdominal skin and wide stretch marks after childbirth
- People whose inner arms and inner thighs have loosened with age and make clothes uncomfortable
- People who had liposuction but still have loose skin
Surgery by area and scar placement
Because skin is cut away, a scar is unavoidable. The incisions are planned in places that clothing covers. Scar length depends on how much skin is removed.
| Area | Scar placement | Scar length (general tendency) | Recovery notes |
|---|---|---|---|
| Arm (brachioplasty / arm lift) | Inner or back of the arm, running from the armpit toward the elbow | If the sagging is only near the armpit, a short incision inside the armpit; if it is wider, it extends close to the elbow | Be careful with raising your arms high for 2–3 weeks |
| Abdomen (tummy tuck-style abdominal skin removal) | A horizontal lower-abdominal incision at underwear-line height, plus around the navel | Often continues to both sides of the hips, and is longer when the sagging is large | Walk slightly bent at the waist for 1–2 weeks |
| Thigh (thigh lift) | The groin crease or a vertical line along the inner thigh | If the sagging is mostly high up, the groin crease only; if wider, an added vertical scar toward the knee | Tightness when sitting and walking; watch for the wound opening |
| Lower body (lower body lift) | An incision that wraps around the whole body above the waist and buttocks | The longest of the group | Longest surgery time and hospital stay |
Scars are red and firm for the first few months, then usually fade and soften over about a year. Depending on your constitution they can widen or become raised, so if you have ever had keloids, be sure to say so at the consultation.
When liposuction and a body lift are done together
Fat and skin sagging often go together, so liposuction and skin removal are sometimes done under a single anesthesia. Liposuction reduces the thickness, then the extra skin is removed to refine the contour.
- Advantage: one anesthesia and one recovery, and fat and skin are shaped within the same plan
- Caution: longer surgery and a heavier recovery. To protect blood flow to the skin, the amount of liposuction in the same spot is often limited
- Doing several areas at once (for example arms + abdomen + thighs) can exceed safe limits, so some clinics suggest splitting it into two surgeries
Fat grafting, which moves the removed fat to hollow areas, may also be planned. Which combination suits you is decided after looking at the degree of sagging and your overall health.
Anesthesia, hospital stay and surgery time
A small area such as one arm or thigh is sometimes done under sedation (sleep anesthesia), but abdominal skin removal or several areas together are usually done under general anesthesia. Surgery time ranges from about 2–3 hours for one area to 4–6 hours or more for several areas or combined with liposuction.
For a wide abdomen or lower-body area, a 1–2 night hospital stay is often recommended. Before surgery you have basic tests such as blood tests, ECG and a chest X-ray, and it is important to confirm that an anesthesiologist monitors breathing and blood pressure during surgery.
Drains and compression garment
Drains
- Thin tubes are often placed where the skin was lifted over a wide area, so blood and fluid don't collect.
- Usually left in for a few days to 1 week, and for up to nearly 2 weeks after a wide abdominal procedure; the clinic removes them once the daily output drops.
- You may be asked to record the output at your accommodation. Your manager helps with how to record it and with the visit schedule.
Compression garment
- Helps reduce swelling and fluid buildup (seroma) and keeps the skin attached in its new position.
- Usually worn for 4–6 weeks, and often 6–8 weeks if liposuction was done as well.
- If the garment presses too hard on the scar area or the skin feels crushed, tell the clinic right away.
Before and after surgery, step by step
- 1Day 1 — in-person consultationthe loose skin is pinched by hand, the removal area and scar placement are marked, and pre-op tests are done. Your manager interprets.
- 2Surgery day — fast, come to the clinic → marking → anesthesia → surgery → recovery room → hospital stay or return to your accommodation
- 3Within 1 week — dressing changes and progress checks, drain check. Drains come out once the output drops
- 4Around 2 weeks — stitch removal and wound check. The schedule may differ if dissolving stitches are used
- 5Before flying home — at the last visit the wound and fitness to fly are checked, and you are given compression garment and scar care instructions
Length of stay in Korea — longer than for liposuction
For liposuction alone, we usually recommend 7–10 days or more, but a body lift has long incision lines, drains and a stitch removal visit, so a stay of about 2 weeks is typical. If you are doing several areas or a large one such as a lower body lift, it is safer to allow longer.
- Day 1Arrive → in-person consultation and pre-op tests
- Day 2Surgery → hospital stay or rest at accommodation near the clinic
- Days 3–7Dressing changes and checks, drain removal. Light walks near your accommodation
- Days 8–13Mostly wound checks. Avoid strenuous sightseeing and long walks
- Days 14–16Stitch removal and last visit, then fly home
Flying home and blood clots
Long surgery, reduced movement and a long-haul flight on top of that raise the risk of blood clots (deep vein thrombosis). A body lift often covers a wider area than liposuction, so prevention matters even more.
- Walk often starting the day after surgery. Don't lie down for long stretches
- On the plane, stand up and walk every 1–2 hours, or move your ankles up and down often while seated
- Drink plenty of water and avoid alcohol on the flight
- An aisle seat makes it easier to move. Don't lift heavy bags; get help from your manager or airport service
- If you have a clot history, take hormone medication or are overweight, talk to your doctor before leaving about prevention (medication, compression stockings, etc.)
- After you get home, go to the local ER right away if one leg swells and hurts, or you are short of breath or have chest pain
Risks to know and symptoms to report right away
Common risks and others to know about
- Seroma (fluid buildup): fluid collects under the skin and feels sloshy. It may need to be drained with a syringe
- Wound opening: most likely where the pull is greatest (middle of the abdomen, groin, near the elbow). Most close with dressing care, but healing can be slower
- Wider or raised scars, and differences between left and right
- Reduced sensation (often recovers over several months), infection, hematoma
- After thigh or arm surgery, swelling in the legs or hands can last for a while
Symptoms to tell your manager right away
- Fever, or the wound getting more red, hot and painful
- The wound opens, or discharge suddenly increases
- Swelling that keeps growing on one side only
- Shortness of breath, chest pain, or swelling and pain in one leg (emergency)
Preparing for surgery and aftercare
- Stop smoking at least 2 weeks before surgery. Nicotine reduces blood flow to the skin and slows wound healing.
- Blood-thinning medicines and supplements such as aspirin, omega-3 and vitamin E should be adjusted only after talking to your doctor
- For 1–2 weeks after abdominal surgery, walk slightly bent at the waist, and sleep with your upper body and knees slightly raised
- After arm or thigh surgery, avoid movements that pull hard on the wound (raising arms high, spreading legs wide) for 2–3 weeks
- Avoid saunas, soaking baths and swimming until the wound is fully healed, and alcohol and hard exercise usually for 4–6 weeks
- After stitch removal, keep up scar care such as scar tape or silicone gel, and sun protection, for several months
Questions to ask at your consultation
If you come from overseas you may get only one or two chances to consult. Take notes of the questions below with you. Your manager will interpret.
- In my case, is liposuction alone enough, or do I need skin removal?
- Exactly where will the scars be and how long? Will clothes cover them?
- If I combine it with liposuction, where and how much will be removed, and how many areas are safe at once?
- Please tell me the type of anesthesia, the surgery time, and whether I need to stay in hospital.
- How many days are the drains left in, and when do the stitches come out? How many days do I need to stay in Korea?
- How high is my blood clot risk, and how do I prevent it on the flight home?
- If the wound opens or fluid builds up after I go home, how will it be treated?
- When should I start scar care, with what, and what are the criteria for a revision?
- Will the doctor I consult with do my surgery personally?
How to choose a clinic
- A board-certified plastic surgeon consults and operates personally and has plenty of experience with skin lifts and body contouring
- For general anesthesia, an anesthesiologist is present, and there are a hospital ward, a recovery room and emergency preparedness
- The scar location and length are drawn directly on your body and explained before surgery
- They don't push you to do too many areas at once and offer a staged plan
- Operating room CCTV, and photo follow-up checks after you return home, are available
KBEAU connects you with partner clinics in Gangnam and Apgujeong led by board-certified plastic surgeons. Send photos of the loose areas, your height and weight and how your weight loss went, and we will tell you in advance the clinic's opinion on whether liposuction alone will do or a skin lift is needed, and how many days you need to stay. A dedicated manager accompanies you through consultation, surgery and follow-up visits, and we can also help book a recovery hotel near the clinic and an airport pickup car.
